Repeat Prescriptions


Your doctor may agree to let you have a prescription for long-term treatment without need for a consultation every time. Please use the following methods for ordering. Use the tear-off slip attached to your prescription or write out your order and:

  • hand it in at the surgery or chemist (allowing a minimum of 48 hours for the prescription to be processed)
  • online via a the link at the top of this page
  • Post the request to the surgery (allowing a minimum of 48 hours for the prescription to be processed.)
  • Requests will NOT be taken over the PHONE at the SURGERY unless there are exceptional circumstances.
  • Telephone  your local chemist e.g. Cookham Pharmacy (01628 521369)

The tear-off slip should be carried with you so that any doctor attending can quickly see what medication you are taking. Please use the sheet to obtain another prescription by either handing it in or sending it to the Medical Centre,  Instructions are on the sheet.

At the bottom of the repeat slip is a review date. This is an approximate date when you should see your doctor to have your medication reviewed. Please make an appointment.

If you are elderly or disabled you can telephone for a repeat prescription on 01628 524646.

Please note that prescriptions are delivered automatically to Cookham Pharmacy, unless you request otherwise. Please write 'Collect from Medical Centre' on the form.

You can now nominate your preferred chemist or appliance provider (if you use one) to receive your prescription by electronic transfer from the surgery so there is no paper copy to collect. This can be your local pharmacy or you may find it more convenient to use a pharmacy near your workplace or a supermarket based pharmacy.  Please ask your preferred pharmacy about Electronic Prescribing (EPS).

Online System

To set up an account the patient will need to come to the surgery and request to sign up, along with some photographic ID e.g. passport driving licence etc. Reception will check the ID and print out your log in details
